This Statement describes the privacy practices for www.evolveinsurancegrp.com and applies only to the online activities of EIG.  EIG may have affiliates, units, and activities, some of which may have individual representation on the web, and may therefore have separate web privacy statements. Collection, Use and Sharing of Information for Insurance Services

EIG collects information about customers necessary to provide services usual and customary to insurance agents. EIG collects this information from the following sources:

  • Information from you on applications or other forms; including, but not limited to, your name, address, social security number, and date of birth.

  • Information about prior transactions with EIG; including, but not limited to, claims and payment history.

  • Information from a consumer-reporting agency; including, but not limited to, a driving record or Insurance Score report.

To collect customer information from the above-stated sources, EIG may use any or all of the following means: written, in-person, telephone, facsimile, electronically, and online.

EIG does not share PII about our current or former customers with non-affiliated third parties other than as permitted or required by law. For example, EIG may share any of the information listed above with non-affiliated third parties for, including but not limited to, the service or process of insurance needs of our customers, in a manner that is consistent with the usual and customary services provided by insurance agents, including, but not limited to:

  • Underwriting, shopping the renewal, rating, placement, and providing quotes for insurance that is germane to the coverage the customer places with our agency.

  • Information that is necessary to protect the confidentiality or security of our customer's record

  • Information that is necessary to resolve customer disputes or inquiries

  • Information that is required by individuals or entities that are assessing our legal compliance.

  • Information that is required to comply with the law.

  • To an actuarial or research organization for the purpose of conducting actuarial or research studies

  • Information that is necessary to protect against or prevent fraud, unauthorized transactions, claims, or other liability

Collection, Use and Sharing of Information Online

EIG does not require you to identify yourself while using our public web pages. EIG public web pages do not collect your Personally Identifiable Information (PII), but we may automatically collect and store general information about you and your visit that does not identify you personally.  This information is collected to track the use of our website to make improvements, and includes, but is not limited to, the following:

  • The IP address from which you access our website

EIG may offer opportunities for you to opt-in to receive information from non-EIG entities. Information Protection

This site has reasonable security measures in place to help protect against the loss, misuse, and alteration of the information under our control. Grievance Process

If you believe that EIG has failed to provide you with appropriate services, assistance, or discriminated against you in another way based on race, ethnicity, color, national origin, sex, sexual orientation, age, or disability, you can file a grievance. Grievances must be filed with Pennie or the Department of Health and Human Services within sixty (60) days of the date you became aware of the alleged discriminatory action. Grievances must be in writing, they must contain the name and address of the person filing the grievance, they must state the problem or action alleged to be discretionary, and the remedy or relief sought.
